Roles and Responsibilities

  • Provide direct clinical counseling services (individual, group, and family) to an assigned caseload.
  • Serve as clinical team lead for assigned QMHP, CSSP, and LPC-Associate staff, providing day-to-day clinical guidance within the scope of licensure.
  • Review clinical documentation (progress notes, treatment plans, assessments) completed by team members for regulatory, clinical, and documentation compliance prior to billing submission.
  • Conduct documentation compliance audits and chart reviews, and provide written feedback on required corrections consistent with agency QM/UM standards and Medicaid documentation requirements.
  • Identify documentation compliance trends across the team and report findings to the Clinical Compliance Director.
  • Provide clinical and crisis consultation to team members as needed.
  • Participate in interdisciplinary treatment planning and case staffings.
  • Ensure timely completion and submission of quarterly reports, progress notes, and discharge summaries for the assigned team.

Compliance Requirements

  • Must hold an active, unrestricted LPC license (independent, non-associate) issued by BHEC.
  • Must pass criminal background, CPS registry, and abuse/neglect registry checks.
  • Must maintain professional liability insurance.
  • Documentation compliance reviews must be conducted consistent with agency QM/UM policy and applicable documentation-of-service-provision standards under 26 TAC Chapter 301, Subchapter G — specific section reference should be confirmed before finalizing.

Minimum Qualifications

  • Master's degree in counseling or a closely related field.
  • Current, active, independent LPC license in good standing with BHEC (associate licensure does not qualify for this role).
  • Prior post-licensure clinical experience required; HR/leadership to confirm minimum years threshold.
  • Experience conducting documentation or chart review preferred.

Knowledge, Skills, and Abilities (KSAs)

  • Strong working knowledge of clinical documentation and Medicaid billing-support standards.
  • Chart audit and QM/UM review skills, including ability to identify and communicate compliance gaps.
  • Comfort delivering constructive, corrective feedback to peers and less-experienced clinicians.
  • Solid DSM-5-TR and treatment-planning knowledge.
  • Organizational skills to manage a personal caseload alongside review responsibilities.
  • Proficiency with Notenetic or comparable clinical documentation platforms.

Employment Classification

  • This position may be filled as a W-2 employee or as a 1099 independent contractor, depending on business need, scope of engagement, and degree of direction and control involved.
  • If engaged as a 1099 independent contractor, the individual must establish and maintain a formal business entity (DBA or LLC) with its own business EIN, and must submit all required credentialing documentation, prior to contract execution and service delivery.
  • Employment classification is determined by Breakthrough Behavioral in accordance with IRS common-law and Texas Workforce Commission standards, and is subject to periodic review.
  • Team-lead and note-review duties involve oversight of other team members' clinical work; if a 1099 arrangement is under consideration for this role, route to counsel for classification review before extending an offer.
